Heart Success Clinic
The team at the Heart Success Clinic helps heart failure patients manage their health after discharge from the hospital.
- Patients are followed in the Heart Success Clinic weekly for a minimum of 4 visits or until medically stable
- Pharmacy, Nutrition and Social Work are available as needed
- Upon discharge from the Heart Success Clinic a follow-up appointment will be arranged with patient's primary care provider, and a progress report will be sent
- Patient education videos are available via television or iPad
