We are experiencing a network disruption incident at Signature Healthcare and currently the Patient Portal System is down. Signature Healthcare is working to resolve this issue as soon as possible.


Signature Healthcare is pleased to offer a state-of-the-art patient portal system. This interactive web portal offers our patients free online access to their personal healthcare. MyHealth empowers patients and their families to take a more active role in their care by providing easy, secure access to their health information and online communication with Signature Healthcare’s physicians and staff.

Signature Healthcare’s Patient Portal is getting a fresh new look!

Starting in early March 2026, users will see a redesigned portal with simpler navigation and easier access to recent health information right from the home dashboard.

The new MyHealth application automatically adjusts to fit your screen, so it’s easy to use on a phone, tablet, or computer.

No action is needed—just log in and explore the enhanced portal.

MyHealth provides patients instant access to their laboratory and radiology results, medications, allergies, visit history and discharge information. Patients can request appointments and medication refills, access their billing statement and outstanding balances and submit electronic payments anytime and almost anywhere.

  • If you are experiencing a life-threatening emergency, please call 911.
  • Messages to your care team should be short and concise (less than 100 characters).
  • Questions requesting medical advice should be addressed directly with your clinical provider. Please message or call to make a virtual or in-person appointment.
  • Please allow up to 72 hours for a reply – excluding weekends.

How to register

Registration for MyHealth is not automatic. You must enroll in the new portal.

To sign up, call your provider's office, contact the portal team at 855-6-PORTAL (855-676-7825) or email MyHealth@MySignatureCare.org.

Patients who used the previous portal, Follow My Health, will always be able to retain their healthcare historical record on that portal.
