All Signature Healthcare, Signature Medical Group Primary Care practices have been awarded Patient-Centered Medical Home recognition from the National Committee for Quality Assurance (NCQA).
NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care supporting access, communication and patient involvement. It assesses whether clinician practices are using teamwork and technology to deliver coordinated and patient-centered Primary Care.

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians, American Academy of Family Physicians, American Academy of Pediatrics, American Osteopathic Association and others.

The Lown Institute recognized Signature Healthcare’s excellence in Patient Safety as #57 in the Nation and #1 in Massachusetts. They also recognized the organization for:

• National Health Equity: Grade A
• National Cost Efficiency: Grade A
• National Inclusivity: Grade A

The recent distinctions align with Signature Healthcare’s record of safe and outstanding quality care. Signature Healthcare Brockton Hospital was awarded its 18th consecutive safety ‘A’ grade from Leapfrog in 2021, an achievement reached by only four hospitals in Massachusetts and 39 hospitals nationwide. The Hospital was recently awarded a High Performing Hospital recognition for the treatment of COPD and kidney failure. Signature Medical Group was again named to the Harvard Pilgrim Physician Group Honor Roll.
